The case manager is a social worker who communicates with the clients in any situation. They are the key person between the company and the clients. A case manager plays a pivotal role in the organization in society. Truly, defining a case manager in a simple word is a very difficult task. There is no single recognized definition of the case manager. But in a quick search, one can define a case manager, a person who collaborates plans, implements, coordinates, monitors, and evaluates the service required to meet clients.

The main role of a case manager is to facilitate Case management. But a good case manager has to be a people person. The case manager must be able to communicate with the people and also listen to their clients. A case manager is like an advocate and must be a mediator in this field. His main job to accumulate all the information he gets from the clients. As a social advocate, he also helps his clients in making a life decision. As a case manager, he needs to do meetings with his clients, do all-important paper works, meeting with the members of the care team, assisting in the making of care plans for the clients, making a daily home visit in delicate issues, and also advocating for the clients.

A case manager carries a variety of responsibilities in a day. But he mainly works on four components to provide the best opinion to their clients.

First, he does a meeting with the clients. The case manager usually uses this time to gather information about the client.

While communicating with the client he tries to identify the immediate needs and begin the relationship with the respective client. This first meeting is very helpful for the case manager to denote the client benefit from the service which he is providing on behalf of his organization. If the providing services are helpful for the client the case manager move on to assessing the client’s individual needs.

Second, need assessment is one of the main jobs of the case manager. He makes this need management on the collective information of his intake stage. The case manager also tries to understand the client’s challenges and problems. During this stage, his primary objective is to identify a client’s interests and the need for success. Each client goes through this stage but when they first come to the organization, it is very important to reassess the needs of the client. The circumstances often change due to any reason. But the case manager always tries to recognize the problems of the client.

Third, Service planning is one of the main jobs of a case manager. This work part is very important to the success or failure of a client. A case manager builds specific goals and takes the necessary actions to set the goals. The main orientation of this goal-setting process is a case plan inclusive of outputs and the outcomes that will measure the success of the client. For a case, a managed service plan is very essential for achieving the goals.

Fourth, understanding the impact of specific programs is very important for a case manager. As a case manager one should continuously monitor and evaluate the client’s progress. He needs to communicate with the client on each topic to get positive outcomes.

So each of these components is important for a case manager. A case manager’s work depends on the information gathering and continuous monitoring of the process. For any organization, a case manager plays a key role for achieve success in the social sector. But for the better impact case manager has coordinate with other providers to ensure the client needs being made.
